there's one patient

came in to SOPD ward with the complaint of anterior neck swelling 2 month duration. he had history of pulmonary TB before and complete the treatment for TB. on futher questioning, he did not have any obstructive symptoms such as dysphagia and dyspnoea. he appeared to have difficulty in speaking but denied any voice changes.
so at first look........we suspected he has an enlarged thyroid.
then, we try to rule out some diagnosis. asking him any symptoms of hyperthyroid or hypothyroid. and then we arrived at this question......

dr: pakcik, ada masalah turun berat badan tak?
pakcik: ada doktor
dr: turun yang macam mane tuh? mendadak ke turun pakcik?
pakcik: mula2 pakcik timbang....berat pakcik 74kg. pahtu pakcik timbang pulak lepas tuh. berat pakcik turun. masa timbang, berat pakcik 73.8kg.
we: *alahai pakcik

well, practically there are some weight lost but if only 0.2kg, i dont think that bring any significant to the current condition. maybe ralat je kot.

but, thanks pakcik. i know he worried about the swelling and try to help us by giving any info that can lead to the diagnosis. it just...............
